The research activity of Prof. Malaguarnera has developed following different themes of General Pathology, Immunology and Oncology.

In the first period of his scientific activity he dealt with the molecular mechanisms involved in the regulation of the activation of monocytes / macrophages. Subsequently, he conducted studies on the regulation of normal hematopoiesis and the mechanisms of altered proliferation of leukemic cells and on the innovative application of gene therapy as an effective anti-tumor means obtaining unpublished data that allowed it to be co-author of scientific papers published on Science, Journal Experimental Medicine, Journal Clinical Investigation, Cancer Research, etc. He also conducted studies on the involvement of apoptotic process regulators during neoplastic events. She has dealt with angiogenic factors that modulate the neoplastic process in relation to the phenomenon of metastasis. He has conducted studies on immune alterations in the elderly and the consequences of immunostaining on the onset of degenerative and neoplastic diseases.

In the immunological field he studied the alterations of immune mediators during Plasmodium Falciparum Malaria infection, as attested by the scientific papers published on Lancet Infectious Disease, Cytokine, Eur. Cytokine Netw, Parasite Immunology. The scientific interests of recent years are focused:

- On the diagnostic and prognostic significance of the Chitinase family in chronic degenerative diseases, neoplasms and metabolic diseases.

- The role of Vitamin D3 and its receptors in immunity and neoplastic processes.

- The role of OAS genes in viral infections and in response to interferons.

On these issues, Prof. Malaguarnera is the author of over 100 publications in peer-reviewed international scientific journals
